File a Police Report Online

Using our online police report system allows you to email a report immediately and print a copy of the police report. Please note the following:

You CANNOT file a report online if:

  • This is an Emergency. (Please call 911)

  • You are reporting a theft or loss of prescription medication. As noted below, you must directly contact Van Wert County Sheriff’s Office for this.

  •  This is a Motor Vehicle Accident.

  • This incident occurred OUTSIDE of Van Wert County or within Van Wert City Limits, please contact the Van Wert City Police Department, non-emergency number (419) 238-2462.

  • You are reporting a theft offense, and the value of the stolen property is more than $1,000.

  • You are reporting a theft offense, and the stolen property is a firearm (gun), license plates, a stolen motor vehicle, or hazardous material.

  • You are reporting Property Destruction, and the damage is valued at more than $1000.

  • There is evidence to indicate that this is a hate crime or bias-related.

  • You are reporting Domestic Violence, or a (Physical Assault).

  • There is any physical evidence connected to the crime that needs to be collected (i.e. blood, obvious fingerprints).


 If you answered “Yes” to any of the above statements, you CANNOT file your report online; you must call the Van Wert County Sheriff’s Office NON- Emergency number 419-238-3866 or 911 for emergencies, where you will speak with a deputy who can either take your complaint over the phone or respond to take a report.

Open the Form on your computer and type in the text boxes.  Be sure to save your work!
If you cannot complete the form online, print it out.

Submit the Form
Once complete, you can submit it one of three ways:
